Fail-closed operating controls

Make the safe path the easy path.

A quality gate is a real pause, test, permission boundary, or receipt. “The model should be careful” is an instruction—not a control.

The six gate types.

  1. 01

    Input gate

    Reject missing, stale, unauthorized, malformed, duplicated, or sensitive inputs before generation.

  2. 02

    Output gate

    Require schema, source trace, allowed values, factual checks, and risk flags before the result moves.

  3. 03

    Human gate

    Pause before sends, publication, client decisions, payments, deletions, and material record writes.

  4. 04

    Permission gate

    Give each step only the minimum read/write scope, target, time window, and volume required.

  5. 05

    Receipt gate

    Advance state only after the external system confirms the action and the expected record is readable.

  6. 06

    Recovery gate

    Define retry limits, duplicate protection, rollback, escalation, and a visible failed state.

For each stage, define: precondition, allowed inputs, validation, model-output check, human approval, action permission, idempotency/duplicate protection, external receipt, failure state, retry rule, rollback or recovery, log fields, and owner. Mark any stage that cannot fail closed as BLOCKED FOR AUTOMATION. Return a pre-launch test plan and a weekly exception review.

An image model produces a staged photo and a downstream workflow could publish it to a listing channel.

Input ownership gate; property-lock prompt; pixel/visual QA; virtual-staging label check; human approval; narrow publish target; platform receipt; public image readback; rollback path.
